Uninsured Services

Fireside Family Health

There are some services that we provide here at Fireside Family Health that are not covered under Alberta Health Insurance. These items are usually related to additional documentation that may be required for employers or insurance companies.

Below is a list of the most common uninsured services and the cost associated with them.

  • Blue Cross Billing (RCMP/CAF/VAC) – $ 100
  • Disability Tax Credit and or CPP – $ 100
  • Disability Benefit Service Canada – $ 150
  • Doctor’s Note – $ 30
  • Driver’s Licence Form – $ 80
  • Driver’s Licence Full Physical – $ 120
  • Foster Care Form – $ 80
  • Injections – $ 20
  • Insurance Brief Statement – $ 40
  • Insurance Report – $ 180
  • Insurance Report Long – $350
  • Insurance Report Short – $ 100
  • Insurance/Lawyer Chart Copies – $ 90
  • Lawyer Review Report Interpretation Consultation – $ 500
  • Long Disability Form – $ 125
  • Missed Appointment(s) Fee – $ 50 to $ 150 depending on appointment type
  • MVA Physician Statement – $ 175

  • Pre-Employment Form Only – $ 60
  • Pre-Employment/Test Form and Exam – $ 160
  • Rush Fee for Insurance/Lawyer – $ 100
  • Senior Residence Report – $ 50
  • Service Canada Report – $ 85
  • Travel Consult Fee – $ 60
